Sample Nurse Letter to School Administrators

Your child's health-care professionals can make requests for accommodations. Use this sample letter.

The formal processes for developing educational and school-related plans require statements from health-care professionals about the needs, restrictions and health status of the student. Physicians are typically involved, but nurses and other providers can play key roles, too.

Here is a real-world example from a nurse at Children’s Hospital of Los Angeles. In the letter she details the recommendations for her patient in the school setting.
